Change Program

CHANGE Program Description

The Change Program is a 12 month healthy lifestyle program.  Based on evidence from diet and exercise research, it aims to improve your overall health and well-being. This might include preventing or improving high blood sugar, high cholesterol, or high blood pressure. With the support of health care professionals, you will gradually make changes to your lifestyle by setting goals for yourself to improve your physical activity levels and eat healthy.

The PCN CHANGE program supports you with managing:

  • Healthy eating

  • Physical activity

  • Blood sugars

  • Blood pressure

  • Cholesterol

Register for a program information session

Program information sessions require no referral and give visitors information about all PCN programs and services.
