The Sherwood Park-Strathcona County Primary Care Network (PCN) is
a group of approximately 100 family physicians and over 60 PCN staff (nurses,
dietitians, pharmacists, exercise specialist, mental health clinicians, and
administrative staff). These professionals work with Alberta Health Services and
other healthcare professionals to improve, coordinate, and deliver primary
healthcare services for patients.

Licensed Practical Nurses and Registered Nurses
Our nursing team provides education and support to help you manage your chronic health conditions. Appointments include asking you about your medical history, medications and address any health concerns such as diabetes, high blood pressure, high cholesterol, weight management, stress and/ or sleep issues. Our dynamic team works with you and your family doctor to provide you the most complete, comprehensive and individual care to help you live well.
Registered Dietitians
Our dietitians will asses your current nutrition status in relation to your
health concerns. This includes obtaining a history on health, changes in
weight, previous diets , dietary restrictions and food allergies or
intolerances. They will complete a review of your food and fluid intake over a
period of 3-4 days and offer support on meal patterns, food portions and
choices. Meal plans are not routinely given out as everyone has different food
preferences, lifestyles, intolerances and caloric requirements. Our dietitians
will offer you guidance based on current nutrition practice guidelines that best
support your health. They strive to help you to set realistic dietary goals
that meet your individual needs.
Clinical Exercise Specialists
Our Exercise Specialist will meet with you to provide practical and realistic
education and guidance about exercise and activity specific to your current
goals and health conditions. Whether you have concerns about arthritis,
osteoporosis, diabetes, chronic pain, fibromyalgia or physical limitations, the
Exercise Specialist will provide you with the most relevant information and
support for your current level of readiness and fitness. Long term activity
programs are also available to help you live well.
Pharmacists
Our Pharmacists will meet with you individually to review your current
medications, relevant lab results, medical history and review all of your
medications concerns. They provide a complete review of all your prescriptions
and supplements, making any necessary recommendations. Our Pharmacists
specialize in diabetes management. They will help you get started with new
medications, insulin or adjust your current regime while consulting with your
physician directly to offer you the most complete and comprehensive
care.
